Free LASIK, LASEK & Vision Correction Consultation
Free Consult Patient Instructions
Meet Personally With an Eye Surgeon
At Park Avenue LASEK, every Free Consultation patient is personally evaluated by an eye surgeon. Whether you are considering LASIK, LASEK, PRK, SMILE, ICL, or have been told you are not a candidate because of a thin cornea or another complex condition, our goal is to provide an honest, thorough evaluation.
Many of our patients have been told elsewhere that they are not candidates for laser vision correction. We welcome complex cases and second opinions.
Office Patient Instructions
Dear [NAME],
Your Free Consultation is on MONTH DAY at TIME AM/PM with Dr. Dovzhuk MD PhD / Dr. Chynn MD FACS
Dr D is from Ukraine & has been Dr Chynn’s assistant for 9 years, so she’s helped manage literally thousands of cases, and knows most things. However, her English is intermediate, so please speak slowly and use short sentences. She’s usually in on Mon, Tue & Fri
If you have a complex situation, like you’re a bad candidate for LASIK because of a thin cornea, it’s best that Dr Chynn sees you himself. He’s usually in on Wed & Th, which is our operative day
If you have a long eye history, like you have cataracts or glaucoma, you were booked incorrectly. So please call us back at 212-741-8628 to change your category to “Extended Office Visit”
We’re the ONLY center where EVERY Free Consultation patient sees an EYE SURGEON during their visit. All other LASIK centers only let you see an optometrist or technician
This is substantially better and safer for you, because every few weeks we see someone who only saw an optometrist, who missed the diagnosis of KC or PMD. Optometrists (ODs) don’t go to medical school & their training lasts only 4 years. Ophthalmologists graduate from medical school, so they’re full MDs, and their training lasts a minimum of 8 years.
Dr D & Dr Chynn both have additional training: Dr D got her PhD in retina in Ukraine, & Dr Chynn completed his Fellowship under two of the most famous eye surgeons in the world: Dr Stulting for corneal translation & Dr Waring for LASIK/PRK/LASEK/ICL & Smile
The highest volume LASIK centers only let in NY (Manhattan LASIK) achieves that volume by doing “remote” consultations. But then they aren’t even examining your eyes, obviously. They just review a copy of your latest prescription & say “you’re a good candidate”. Dr Chynn feels very strongly this isn’t the best practice of medicine. This is exactly how ODs miss KC & PMD. If you have one of these conditions & get LASIK, you’ll probably eventually need a cornea transplant, which is replacing your damaged cornea with a cadaver cornea, & will never see normally ever again. You might win a lawsuit & get $1 million dollars, but most people would rather see than be rich & not see
Because we’re setting aside 1 hour of our eye surgeon’s time, it’s inappropriate for you to “no show” for your appointment. Most people cancel hair appointments, because they’re know if they don’t, the hairdresser loses an hour slot and $100/$200. If you don’t bother to call to reschedule, our surgeon will be waiting for you, and we’ll lose $500. So please be polite & call or text this group 1 day before if your plans change. Texting us the same day doesn’t accomplish anything, because your slot will probably remain empty, and we’ll still lose $500
Dr Chynn is the only high-volume laser vision correction surgeon in the NY Tri-State area who performs CXL, which is a way to strengthen your cornea if its thin, or you have KC/PMD. So don’t give up hope! Most people rejected for LASIK can be safely treated at our center
Because Dr Chynn performs CXL & corneal transplants, which are covered by medical insurance, Dr Chynn got credentialed with all major plans (except Medicaid), whereas most other LASIK doctors didn’t. This is better for you even if you don’t have KC/PMD or need CXL, because your medical insurance will pay us about $500 for your exam. So, at the end of your Free Consultation you’ll be asked if you’ll let us submit a claim. If you agree, we’ll take what we receive & subtract that from your cost. Bad plans will pay $100. But union plans (1199, 32BJ, etc) might pay $900!
This is why you’re supposed to send a copy of the front & back of your medical insurance card, so we go on its website to estimate how much it’ll pay. So please do this now. If you wait until arrival to do this, we may not have enough time to determine this (because their customer service takes forever to pick up). We aren’t legally allowed to bill your insurance without your signature, so don’t worry about us doing that without your permission
Dr Chynn is the only eye surgeon in NY, NJ, CT or PA who asks every patient to bring in both their current & old glasses and contacts. So, he’s the only one who is assessing if your prescription is truly stable. Every other LASIK center just asks “is your prescription stable?” which isn’t really safe, because no layperson knows the FDA definition of refractive stability
This will affect your longterm results, because every other LASIK surgeon simply enters your current prescription into the laser & steps on the pedal. So if you’re not totally stable, you’ll see well initially, but see slightly worse every year until your eye stops growing (this happens at age 30)
In contrast, Dr Chynn is so meticulous that he wants to know what your exact profession is (other centers don’t ask) to assess if you’re reading, on the computer, or on your cellphone a lot. He even wants you to tell him if you’re planning on going to graduate school, & if your parents or siblings have extreme prescriptions. Why? Because his published research has proven that these factors cause more myopic progression. In which case he’ll enter a slightly higher prescription than your current Rx, based on his “nomogram” of 35,000 procedures performed over 30 years
Dr Chynn has performed more monovision cases than every other LASIK surgeon in NYC—combined! Why? Because he’s the only one who had that himself, & knows it works (he’s 60 & doesn’t need readers)!
Please be clear that Dr Chynn is a perfectionist, has a Type A personality, is in MENSA (certified IQ over 150) & is a native New Yorker (he was the only MD to go down to Ground Zero on 9/11 itself as a volunteer). So if he’s “strict” with you or his staff, it’s for one reason: because he can’t ensure perfect results unless both you & his staff do exactly what he says. As the saying goes, if you want a friend, get a dog
(Dr Chynn loves dogs
). If you want your eyes lasered by the most meticulous person you’ve ever met, then you’re in luck
Dr Chynn is the only eye surgeon in NYC who’s gotten LASIK himself (in 1999). Every other prominent LASIK surgeon didn’t, and many are secretly wearing contacts
We won’t even hire anyone unless they’re willing to get lasered. So our surgeon & staff can speak from personal experience & a patient’s POV. We don’t say “it doesn’t hurt” or “you shouldn’t get dry eyes.” We say “I KNOW it didn’t hurt” or “yes, my LASIK eye is dry, but my LASEK eye isn’t” (Dr Chynn)
Dr Chynn is the only eye surgeon in the country that creates a private, encrypted WhatsApp group for every patient, & is the only one who guarantees to answer your questions within 24 hours, both before—and more importantly, right after your surgery. He will reply nights, weekiends-even when abroad giving lectures (he’s travelled to 76 countries). But please be respectful of his time. If you keep asking him questions that are already in the 10 pages of FAQs on our website, he might leave your group (his staff will remain)
We’re at 102 E 25th St & Park Avenue South, on the SE corner next to Citibank. Look for our red laser
Watch at least the first 3 patient videos at https://parkavenuelasek.com/stage-directions before your visit to have your basic questions answered.
At the end of your visit, if you wish to proceed, you’ll pay a $500 “testing fee” and we’ll perform your preop testing. This fee goes towards your balance. It’s not refundable because we can’t get that time back
If you’ve made it to this point, congratulations – you’re going to be a “good” patient. Just like Britney our Practice Manager & Constantin our Patient Coordinator – both of whom Dr Chynn made see 20/15, which is BETTER than a “perfect” 20/20 after their HD LASEKS (remember the importance of compliance)We look forward to meeting you in person & giving you your dream of perfect vision—without glasses or contacts

This page contains the same patient instructions our office provides before a free consultation. These instructions explain what records to bring, how insurance may apply, what to expect during the evaluation, and how LASEK benefits patients.
If you have been told you are not a candidate for LASIK , a consultation may help determine whether other treatment options exist.
