Dry Eye Specialty Practice
We are one of only 2 eye centers in NYC that specialize in treating dry eyes
(the other one was started by an eye MD whom Dr. Chynn helped train when he was a Resident Instructor at New York Eye & Ear Infirmary).
If you have dry eyes, Dry Eye Syndrome (DES), contact lens intolerance (defined as the inability to wear contacts at least 12 hours per day without symptoms, or suffer from scratchy eyes, have a frequent “foreign body sensation” of something in your eyes, watery eyes when it’s cold and/or windy outside, you need to use artificial tears more than once per day, you have fluctuating vision, you suffer from “mucus pulling syndrome“, or any eye doctor has said you have dry eyes, we can help!
If you have not had all of the following tests, your diagnostic workup was not thorough, and the doctor might have missed the diagnosis of DES:
microscopic slit-limp exam
fluorescein staining of your cornea and conjunctiva
TBUT (tear break-up time)
Rose bengal and lissamine green staining (where indicated)
assessment of diameter of upper and lower tear puncta (the “drainage holes” of your eyes)
assessment of MGD (meibomian gland dysfunction), which is the #1 cause of DES in Caucasians
And if you have not tried most or all of the following treatment modalities, your symptoms might not go away, because you have not had what we consider “full treatment“:
Hot compresses and lid hygiene/scrubs (for MGD)
Doxycycline pills (for MGD)
specialized device to heat up the lids & express “clogged glands” (for MGD)
preservative-free artificial tears
ophthalmic ointment at bedtime
taping lids closed at bedtime (for lagophthalmos)
temporary collagen plugs (to diagnose excessive tear outflow)
semi-permanent silicone plugs (to treat excessive outflow after diagnosis)
punctal cautery (to permanently fix the problem if your silicone plugs keep falling out)
Restasis or Xiidra (to increase your production of tears–these are prescription drops)
steroid drops (to treat the inflammatory component of DES in more severe cases)
